The nose is an important organ because it protects the entire airway and plays a vital role in respiration. However, laryngectomy and tracheostomy patients lose the benefit of the nasal function when breathing through the stoma opening in the neck. This can lead to discomfort, inconvenience and serious respiratory complications if the functions of the nose are not replaced and adequate airway protection provided.

Before Laryngectomy
The nose warms, humidifies and filters the incoming air. The nasal passages and the upper airway also provide airway resistance during expiration, which is important for efficient breathing. Heat and moisture ensures the mucosa and the tracheal lining are kept in good working order, which is important in keeping the tracheal airway clean and controlling mucus production. Because the airway is intact, normal respiration and efficient gas exchange take place within the lungs.

After Laryngectomy - the missing Nasal Function
The laryngectomee breathes through the stoma in the neck, bypassing the nose. Therefore air is not being properly warmed, humidified and filtered. Cold, dry, unfiltered air is entering the lungs. There is no protection from the environment. Dust, smoke and bacteria will enter the trachea. There is no airway resistance provided so there will be a reduction in respiratory performance, possibly leading to difficulties and discomfort.
Complications arise when the airway is not protected. Lack of heat and moisture means that the tracheal mucosa will dry out. Crusting will occur; excessive mucus will be produced. Cold, dry and unfiltered air enters into the lungs, coughing is likely to increase and chest infections often occur.
Lack of airway resistance and therefore poor gas exchange, will diminish the respirator function, which can contribute to people feeling tired and lethargic.

But Trachi-Naze restores the lost nasal airway function and benefits of the nose.

Trachi-Naze is the first system designed specifically to improve the respiration of the neckbreather. It restores the nasal function as it is the only externally worn system to add back heat, moisture and anti-bacterial protection It provides functional airway resistance within the trachea and thus improves respiration by increasing oxygenation. Many laryngectomees wear stoma covers and protectors, but these open systems tend to be loose fitting over the stoma and can only offer basic protection. There are a variety of products which will offer heat and moisture protection, but none will offer the combined levels of anti-bacterial filtration and increased oxygenation and improved respiration provided by Trachi-Naze.

THE TRACHI-NAZE SYSTEM PROVIDES A CHOICE OF THREE LEVELS OF AIRWAY RESISTANCE

in Multi-layer discs featuring activated carbon, non-woven hydrophyllic layer and viscose micro filter layers.

Normal airway resistance is provided the Night filter (blue filter/blue product pack for ease of identification.
Half the normal level of airway resistance is created by the Day filter for comfortable wear without any feeling of constriction during daily activities (green)
Minimal airway resistance is provided by the Active filter. (orange) This allows the wearer to participate in more strenuous activity without restricted breathing - e.g. running, cycling, gardening.

The filter discs contain a pre-filter to filters out dust and larger airborne particles, an activated carbon layer to prevent bacteria from entering the airway and to further cleanse the air of smoke, gases and other small particles, and a heat/ moisture exchange layer to keep the tracheal mucosa in good condition. In combination, with the baseplate sealed onto the skin around the stoma, the system provides the desirable airway resistance.

A SELECTION OF BASEPLATES LETS YOU CHOOSE A DEVICE THAT WILL BE THE MOST COMFORTABLE AND SUITABLE FOR YOUR OWN UNIQUE NEEDS.
They feature a collar into which a filter disk fits. Each baseplate stays on neck an average of 2 to 3 days, allowing a change of filters as appropriate.

The Hydrocolloid material is a well proven medical adhesive system. The Hydrocolloid (gelatin/pectin material), flesh colored baseplates are flexible, light weight and hypoallergenic with an excellent surface for attachment to the neck.

The standard (small) baseplate (#38064) is suitable for smooth, round, flat stomas. The large baseplate (#38065) should be used when more surface area is needed for better adhesion, such as with a voice prosthesis. A protective barrier wipe can be used to supplement the adhesive qualities of the base plates if desired.

A 4th Choice in baseplates (not pictured) is round with a very "aggressive" adhesive for those who find that normal adhesives do not stick well enough. Particularly recommended for TEP voice prosthesis users to maintain a good seal. Air permeable material lets skin breathe. Clear, so user’s skin color shows through. Cat. #38067.

 


 



 
       EXPIRATION AIRFLOW
The Trachi-Naze pressure relief system avoids discomfort from build- up of pressure The disk fits loosely into the baseplate collar, enabling the filter to move outward during expiration, so air can escape and release the pressure behind the filter.

The flesh colored polyethylene OCCLUSION CAP secures the disk to the baseplate collar with a simple twist/turn motion. The round opening allows easy finger occlusion when wearing a voice prosthesis. Note: The current Trachi-Naze is not suitable for use with an automatic valve.

IDEALLY, SO THAT THE RESPIRATORY SYSTEM WILL EXPERIENCE THE LEAST POSSIBLE NEGATIVE EFFECTS, USE OF THE TRACHI-NAZE SHOULD BEGIN AS SOON AS THE SKIN IS SUFFICIENTLY HEALED AFTER SURGERY. STARTING TO USE THE SYSTEM AT A LATER DATE MAY TAKE A LITTLE MORE TIME AND "PRACTICE" TO ACHIEVE COMPLETE COMFORT AND BEST RESULTS, BUT THE RESPIRATORY ADVANTAGES SHOULD BE WELL WORTH THE EFFORT.
